Up-skill your capabilities – how to apply

Nurses and midwives wishing to apply for a program must submit the following with their completed application forms:

• Current registration with the NMBWA
• Approval from your manager to undertake program if currently a WA Health employee
• Copy of curriculum vitae

• Paediatric up-skilling program application form (Word 200KB)
• Midwifery up-skilling program application form (Word 205KB)
